Job summary

Peace and Harmony Homecare LLC seeks a Physical Therapist to provide home-based PT services in collaboration with primary care physicians and the health care team. You will educate patients and families, assess function, and develop an effective Plan of Care for ongoing therapy.

The role requires CAPTE-accredited education, an active PT license, CPR certification, and at least 1 year of PT experience. You will travel to patient residences and ensure timely documentation and communication with

Qualifications

• Must graduate from a CAPTE-approved PT program.
• Active PT license in good standing.
• Minimum 1 year experience as a Physical Therapist.
• Criminal background check and current CPR certification.
• If trained outside the United States, meet eligibility requirements per World Confederation for Physical Therapy guidelines.

Responsibilities

• Receives intake referral information and prepares PT paperwork/tools for visits.
• Obtains medical history from patient, family, or caregivers.
• Performs PT assessment and documents findings.
• Conducts home safety and environmental assessment of patient’s home.
• Assists physician in evaluating functional level and develops/updates Plan of Care.
• Reassesses patient for ongoing therapy and evaluates medical necessity for other services.
• Evaluates patient’s ADL and plans for Home Health Aide services when applicable.
• Supervises Home Health Aide as required by state/federal guidelines.
• Communicates with patient and family about POC and progression.
• Instructs patients and families on use of prosthetic/orthotic devices.
• Communicates with other disciplines and physician to optimize treatment.
• Submits Oasis assessments within 48 hours and CRR monthly.
• Participates in Performance Improvement Committee and staff meetings.
• Contributes to office growth and acts as preceptor for new hires.
