Job summary
Peace and Harmony Homecare LLC is seeking a licensed Physical Therapist to deliver professional PT services in collaboration with physicians and the health care team. You will educate patients and families, perform assessments, and contribute to home-based therapy plans.
The role requires CAPTE-approved PT education or equivalent, a current US PT license, at least 1 year of PT experience, and CPR certification. Home visits and coordination with care teams are essential.
Qualifications
- Must hold a current license to practice Physical Therapy in the United States.
- Graduate of a CAPTE-approved PT program or equivalent.
- Minimum of 1 year of PT experience.
- Criminal background check and current CPR certification.
Responsibilities
- Prepare PT paperwork/tools from intake referrals.
- Obtain medical history from patient and family.
- Perform PT assessment and document findings.
- Develop and submit Plan of Care (POC) to physician and update every 30 days.
- Supervise Home Health Aide when PT is the only skilled discipline.
- Communicate treatment plans with patient, family and physician.
- Submit all OASIS assessments within 48 hours of visits.
- Perform clinical record reviews with Nursing Supervisor.
