Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private homes and senior living communities across Massachusetts. You will evaluate, design plans, and deliver evidence-based therapy, with telehealth options and flexible scheduling.

The role emphasizes autonomy, patient outcomes, and collaboration with a multidisciplinary team, with opportunities for career growth and modern clinical tools.

Qualifications

• Valid Physical Therapy license in the state of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program (DPT preferred).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Document therapy in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, and Fitness Specialists).
