Job summary

FOX Rehabilitation in West Roxbury, MA seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes or senior living communities. You’ll conduct evaluations, create individualized plans, and provide evidence-based therapy while documenting in FOX’s EMR.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Perform evaluations, develop plans of care, deliver evidence-based treatment
• Deliver therapy sessions to support mobility, independence, and functional goals
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access
