Job summary

FOX Rehabilitation is expanding its Concord, MA team and seeks licensed physical therapists who want to deliver one-to-one care to older adults in private homes or senior living communities. You will manage schedules, form patient relationships, and may incorporate telehealth for flexibility in care delivery.

Qualifications

• Licensed Physical Therapist in MA or eligible to obtain licensure
• Experience delivering in-home or senior living care preferred
• Strong communication and documentation skills

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Develop and implement individualized plans of care and deliver evidence-based treatment
• Document patient progress in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
