FOX Rehabilitation is seeking licensed Physical Therapists in Wilmington, MA to deliver one-on-one geriatric care in patients’ homes or senior living communities. Therapists will assess, plan, and execute personalized therapy programs to improve mobility and independence.

The role offers autonomy in scheduling, the option to incorporate telehealth, and opportunities to collaborate with a multidisciplinary team, enhancing continuity of care for older adults.

Qualifications

• Active Physical Therapy license in MA or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Current CPR certification

Responsabilités

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care
• Deliver therapy sessions to improve mobility and independence
• Document in EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients and caregivers to promote aging in place
• Provide telehealth/virtual care when appropriate
