Job summary

FOX Rehabilitation in Burlington, MA is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences and senior living communities. You’ll assess mobility, create individualized plans of care, and improve function through evidence-based treatment, all while managing your own visit schedule.

The role offers autonomy, flexibility, and the opportunity to use telehealth as part of the Geriatric House Calls model, with a supportive, clinician-driven culture

Qualifications

• CPR certification required.
• EMR proficiency demanded.
• PT license in state of practice or eligibility to obtain.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and functional independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote aging in place.
• Provide telehealth/virtual care when appropriate.
