Job summary

Powerback is seeking a licensed Physical Therapist to deliver outpatient rehabilitation services in patients' homes across the region. You will design and implement individualized treatment plans, collaborate with the rehab team, and ensure high-quality care that supports independence for older adults.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Bachelor's degree with 5 years of PT experience.
• Driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in home settings for older adults.
• Develop and deliver individualized PT programs and coordinate referrals to address patient needs.
• Educate patients and caregivers on skills for independence and mobility.
