Job summary

Powerback Rehabilitation seeks a licensed Physical Therapist to provide outpatient rehabilitation in-home settings for older adults. You will evaluate, plan, and deliver therapy programs in patients’ homes or in independent living facilities, coordinating with the care team to restore function.

The role emphasizes individualized treatment, caregiver education, and safe home-based care across diverse environments.

Qualifications

• Must hold a valid Physical Therapy license in practice state(s).
• Master's degree in Physical Therapy or PT-related field with Bachelor's degree required.
• Bachelor's degree in Physical Therapy with 5 years of PT experience acceptable.
• US degree evaluation for non-US credentials if applicable.

Responsibilities

• Supervise PTAs, temporary licensees, students and staff as allowed by licensure.
• Adhere to department schedules, clinical and billing documentation requirements.
• Attend patient care and related staff conferences.
• Make referrals to other disciplines to meet patient needs.
• Collaborate to select appropriate therapeutic regimens for patients.
• Adapt plan of care for individual patient needs.
• Promote a safe environment and manage equipment requirements.
• Provide education to patients and caregivers for home programs.
