Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based therapy to adults with developmental disabilities across New Jersey. You will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions under NJ DDD guidelines.

Qualifications

• Current New Jersey license as a Physical Therapist (PT).
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean background check and fingerprinting per NJ DDD requirements.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services per DDD standards, including notes and outcomes within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's/Stephen Komninos' Laws, etc.).
