Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities across New Jersey. You will evaluate mobility and functional movement, develop individualized plans, and work with families and coordinators to maximize independence and community participation.

Responsibilities include conducting assessments, delivering one-on-one therapy in homes, documenting progress, and ensuring compliance with DDD

Qualifications

• Current New Jersey license as a Physical Therapist (PT) issued by the NJ State Board of Physical Therapy Examiners.
• CPR/BLS certification is current and acceptable for clinical practice.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in compliance with DDD standards and required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Maintain HIPAA confidentiality and adhere to DDD policies and trainings.
• Maintain accurate billing records consistent with DDD rates and authorizations.
• Complete required DDD-mandated trainings.
