Job summary

Powerback Rehabilitation seeks a licensed Physical Therapist to provide in-home outpatient rehabilitation for older adults. You will evaluate patients, design individualized therapy programs, and implement plans of care in patients' homes or independent living settings.

The role emphasizes patient education, collaboration with the rehab team, and documentation. A strong focus on safety, quality care, and enhancing patient independence is required.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy; or Master’s in a related field plus Bachelor’s in Physical Therapy.
• Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Evaluate, develop, and deliver individualized physical therapy programs.
• Educate patients and caregivers to promote independence.
• Refer to other disciplines as needed and coordinate care with the rehab team.
• Maintain documentation and schedule adherence as required.
