Job summary

Iowa Home Care is seeking a licensed Physical Therapist to perform in-home evaluations and treatments in the West Des Moines area. You will assess mobility, balance, and safety, then design and implement individualized plans for patients at home.

The role emphasizes collaboration with a multidisciplinary team and education for patients and families, with flexible scheduling and opportunities to impact care at the community level.

Qualifications

• Licensed to practice as a Physical Therapist in Iowa.
• Minimum of two years of experience in physical therapy; community/home health preferred.
• Strong communication, organizational, and problem-solving skills.
• Valid driver's license with reliable transportation.

Responsibilities

• Conduct in-home physical therapy evaluations.
• Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety.
• Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care.
• Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards.
• Recommend adaptive equipment and home modifications when appropriate.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate changes in condition or concerns to the interdisciplinary care team.
• Promote independence and ongoing improvement through compassionate, patient-centered care.
