Job summary

Innovive Health is seeking a licensed Physical Therapist to join our in-home care team in Iowa City. You will perform evaluations, develop personalized treatment plans, guide patients through therapeutic exercises, and participate in discharge planning to ensure continuity of care.

The role requires strong communication, local travel to patient residences, an Iowa PT license, CPR certification, and a valid driver's license.

Qualifications

• Licensed Physical Therapist in the state of Iowa.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Home health experience required.
• Strong communication, organization, and clinical decision-making skills.

Responsibilities

• Provide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and discharges.
• Develop and implement personalized treatment plans based on patient needs and physician orders.
• Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement.
• Educate patients and caregivers on safety, fall prevention, and daily living strategies.
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
• Assess vital signs as needed and communicate any concerns to the care team.
• Recommend and educate on the use of assistive devices and durable medical equipment (DME).
• Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians.
• Complete timely and accurate documentation.
• Support discharge planning and ensure continuity of care.
