Job summary

WesleyLife Hospice is seeking a licensed Physical Therapist to provide in-home, one-on-one care across Iowa. You will assess mobility, design personalized treatment plans, and deliver targeted therapy to restore function and independence.

You will educate patients and caregivers, coordinate with the care team, and adjust plans as progress dictates, ensuring compassionate, evidence-based support in the patient’s home.

Qualifications

• Currently licensed as a Physical Therapist in the state of Iowa.
• Current Basic Life Support certification.
• Doctor of physical therapy degree from a physical therapist education program.

Responsibilities

• Assess clients in their homes and develop individualized treatment plans to support mobility, safety, and functional goals.
• Deliver one-on-one therapy using tailored exercises, manual techniques, gait training and evidence-based practices.
• Educate clients and caregivers on safe activities, pain management, and home modifications to support independence.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated, holistic care.
• Monitor progress and adjust plans as needs evolve to ensure high-quality, responsive support.
