Job summary

Iowa Home Care is seeking a dedicated Occupational Therapist to perform in-home evaluations and develop personalized treatment plans to help clients safely perform daily activities. You will work with a diverse interdisciplinary team to deliver patient-centered care and education in the comfort of each patient’s home.

Qualifications

• Licensed to practice as an Occupational Therapist in the state of Iowa.
• Minimum of two years of experience in occupational therapy; community/home health experience preferred.
• Strong communication, organizational, and problem-solving skills.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations.
• Develop and implement individualized treatment plans focused on improving ADLs and IADLs.
• Provide therapeutic interventions to improve upper extremity strength, coordination, cognitive skills, and functional independence.
• Train patients in adaptive techniques to enhance safety and independence with bathing, dressing, grooming, toileting, and meal preparation.
• Recommend adaptive equipment and home modifications to improve safety and accessibility.
• Educate patients and caregivers on home programs, energy conservation, fall prevention strategies, and equipment use.
• Collaborate with nurses, physical therapists, speech therapists, social workers, and case managers to coordinate care.
• Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate changes in condition or concerns to the interdisciplinary care team.
• Promote independence and ongoing improvement through compassionate, patient-centered care.
