Qualifications

• Licensed to practice as a Physical Therapist in Iowa.
• Minimum two years of physical therapy experience; home health preferred.
• In-home evaluations and mobility/strength training required.

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations.
• Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety.
• Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Coordinate with nurses, therapists, social workers, and case managers to ensure comprehensive care.
• Document visits, progress notes, and care plans in compliance with standards.
• Recommend adaptive equipment and home modifications when appropriate.
• Monitor progress and adjust plans as needed.
