Responsibilities

• Provide outpatient rehabilitation services in home and community settings.
• Evaluate, plan, and implement individualized physical therapy programs.
• Educate patients and caregivers to promote independence and functional recovery.
• Coordinate with rehabilitation team to address comprehensive patient needs.
• Ensure timely documentation and adherence to schedules and safety standards.
• Orders supplies and maintains equipment as needed.
