Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based PT services for adults with developmental disabilities across New Jersey. The PT will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and community participation.

Qualifications

• Current New Jersey license as a Physical Therapist (PT)
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent)
• Minimum of one (1) year of clinical experience preferred; new graduates considered
• Current CPR/BLS certification
• Clean criminal background check and fingerprinting per NJ DDD requirements
• Reliable transportation, valid NJ driver’s license, and current auto insurance
• Ability to pass DDD-required background checks including CRoff

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals
• Deliver one-on-one therapy services in home and community settings
• Document services per DDD standards and submit within required timeframes
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team
• Provide caregiver training and home programs to reinforce goals
• Adhere to all DDD policies and HIPAA confidentiality standards
• Maintain billing records consistent with DDD rates and authorizations
• Complete required DDD trainings
