Job summary

BrightStar Care is seeking a licensed Physical Therapist (PT) to join our home health team in Lexington, KY. You will evaluate patients in their homes, develop personalized care plans, and deliver skilled PT interventions to improve strength, balance, mobility, and safety.

We offer a flexible schedule, part-time or per diem options, and a competitive per-visit rate with mileage reimbursement. New grads welcome and Joint Commission-accredited agency.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
