Job summary

BrightStar Care – Concord, Lexington & Woburn seeks a licensed Physical Therapist to perform in-home evaluations and deliver personalized PT plans. Part-time or per diem with flexible scheduling and local visits within the MA area.

Candidate must hold a MA PT license, have a DPT or MPT from an accredited program, CPR certification, and ideally 1+ year of experience. A reliable vehicle and valid driver’s license are required.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program required.
• CPR certification required.
• 1+ year of PT experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and develop personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
