Pathways Healthcare seeks a dedicated Physical Therapist to provide in-home rehabilitation services across our Concord, MA area. You will evaluate patients, develop plans of care, and deliver hands-on therapy aligned with physician orders.

Qualifications

• Degree from an accredited Physical Therapy program.
• Valid MA Physical Therapist license.
• Minimum two years of relevant home health or community experience preferred.
• Excellent verbal and written communication and organizational skills.
• Maintain current BLS CPR certification and a valid driver's license with reliable vehicle.

Responsibilities

• Deliver physical therapy services per physician's plan of care and state law.
• Assess functional status, mobility, gait, and range of motion.
• Administer therapy treatments and instruct on use of devices.
• Document progress and report changes to physician.
• Initiate and manage PT programs, collaborating with care team and family.
• Instruct staff and caregivers on therapy goals and phases.
• Provide training on prosthetic devices and refer for additional services as needed.
• Contribute to discharge planning and case conferences.
