Job summary

Pathways Healthcare is seeking a full-time Physical Therapist to provide in-home care within Concord, MA and surrounding towns. You will evaluate patients, deliver therapist-guided rehab per physician plans, and collaborate with families and caregivers to support recovery at home.

The role emphasizes strong communication, organizational skills, and a current MA PT license with BLS CPR. A valid driver’s license and a reliable vehicle are required.

Qualifications

• Degree from an accredited Physical Therapy program.
• Hold a valid Massachusetts Physical Therapy license.
• At least 2 years of PT experience; home health/community experience preferred.
• Excellent verbal and written communication; strong organizational skills.
• Maintain current BLS CPR certification.
• Valid driver’s license and owned vehicle.

Responsibilities

• Deliver PT services per physician plan of care and state PT Act.
• Assess and evaluate therapeutic, rehabilitative, and functional status in home settings.
• Perform strength, mobility, gait, and ROM assessments for rehab potential.
• Administer PT treatments and instruct on use and care of appliances.
• Report responses to treatment or changes in condition to physician.
• Initiate and manage PT programs; collaborate with other health professionals and families.
• Instruct others on PT phases and treatment goals; train patients on prosthetic devices.
• Identify additional home health services needed and refer accordingly.
• Contribute to discharge planning and case conferences.
• Prepare and submit clinical and progress summaries.
