Job summary

Pathways Healthcare is seeking a licensed Physical Therapist to join our home health and hospice team in Massachusetts. The role involves delivering PT services at patients' homes, evaluating rehabilitative needs, and coordinating with physicians to ensure optimal recovery.

You will manage therapy programs, train caregivers, and participate in discharge planning, with a focus on patient-centered care and safety. Massachusetts licensure and BLS CPR are required, with a valid driver's license.

Qualifications

• Must have a degree from an accredited Physical Therapy program.
• Massachusetts license to practice as a Physical Therapist.
• Minimum two years of PT experience; community/home health preferred.
• Excellent verbal and written communication; strong organizational abilities.
• Current BLS CPR certification and driver's license.

Responsibilities

• Deliver physical therapy services per physician's care plan and MA statute.
• Assess rehabilitative status, including home environment.
• Evaluate muscle strength, mobility, gait, ROM to identify rehab potential.
• Administer treatments and teach use of therapeutic devices.
• Report patient responses and condition changes to the physician.
• Initiate and manage PT programs with healthcare teams and families.
• Educate caregivers on therapy phases and goals.
• Train patients on prosthetic devices.
• Identify additional home health services and refer as needed.
• Contribute to discharge planning and case conferences.
• Prepare clinical progress summaries on goal achievement.
• Provide PT consultations to home families as needed.
• Lead in-service education for nursing staff.
• Engage in peer consultation.
• Supervise PT assistants per policy and state rules.
• When therapy is sole skilled service, supervise and evaluate home health aide care bi-weekly.
