Job summary

Pathways Healthcare, a leader in home health and hospice care, seeks a licensed Physical Therapist to deliver services in patients’ homes across our Massachusetts service area. This role focuses on evidence-based rehab plans, patient education, and family involvement within a physician-guided framework.

Ideal candidates will have at least two years of PT experience, a valid MA license, and BLS CPR certification.

Qualifications

• Degree from an accredited physical therapy program.
• Massachusetts license to practice required.
• Minimum of two years of relevant PT experience.
• Current BLS/CPR certification.

Responsibilities

• Deliver physical therapy services per physician's plan of care.
• Assess and document patients' functional status and home environment.
• Administer therapies and instruct on use of devices.
• Coordinate care with other health professionals and family.
• Assist in discharge planning and care coordination.
