Job summary

Pathways Healthcare, a leading home health and hospice organization, is seeking a full-time Physical Therapist to deliver in-home care under a physician's plan of care in Massachusetts. The role requires a minimum of two years of PT experience (home health preferred), MA PT license, BLS CPR, and a valid driver's license with reliable vehicle.

You will collaborate with the care team and support discharge planning.

Qualifications

• Must hold an accredited Physical Therapy degree.
• Valid Massachusetts Physical Therapist license.
• Minimum 2 years PT experience; home health preferred.
• Current BLS CPR certification.
• Valid driver's license and own vehicle.

Responsibilities

• Deliver PT services per physician plan of care.
• Assess functional status and home environment.
• Perform strength, mobility, gait and ROM assessments.
• Instruct on prosthetic devices and rehab goals.
• Collaborate with care team and discharge planning.
• Supervise PT assistants per policy.
• Provide education to families and caregivers.
