Job summary

Pathways Healthcare is seeking a Physical Therapist to deliver high-quality home health services from our Concord, MA office. The role serves clients across Concord and surrounding communities, including Lowell, focusing on rehab goals in patients' homes.

You will assess, plan, and implement therapy programs, collaborate with physicians and families, supervise assistants, and contribute to discharge planning.

Qualifications

• Must have degree from an accredited Physical Therapy program.
• Massachusetts license to practice as a Physical Therapist.
• Minimum of two years of relevant PT experience; home health/community experience preferred.
• Excellent verbal and written communication skills.
• Maintain BLS CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Deliver physical therapy services per physician's plan of care and the state Physical Therapy Practice Act.
• Assess therapeutic, rehabilitative, and functional status, including home environment assessment.
• Assess muscle strength, mobility, gait, and range of motion; identify rehabilitation potential.
• Administer physical therapy treatments and provide instruction on use and care of therapeutic appliances.
• Report patient's reaction to treatment or changes in condition to the physician.
• Initiate and manage physical therapy programs, collaborating with other healthcare professionals and family/caregiver members.
• Instruct other personnel, family members, and caregivers on PT phases and goals.
• Offer training to patients on the use of prosthetic devices.
• Identify additional home health services needed by patients and refer as necessary.
• Contribute to discharge planning and participate in case conferences.
• Prepare and submit clinical and progress summaries.
• Provide PT consultation to home families/caregivers.
• Conduct in-service education programs for nursing organization personnel.
• Engage in peer consultation process.
• Supervise physical therapy assistants in compliance with policy/state regulations.
• When therapy is the sole skilled service, instruct, supervise, and evaluate home health aide care bi-weekly.
