Pathways Healthcare is seeking a licensed Physical Therapist to provide in-home PT under physician plans across a Massachusetts service area. You will evaluate patients, deliver treatments, and educate families to support recovery at home.

The role is full-time with Monday–Friday schedule from Concord, MA, offering a competitive salary and comprehensive benefits. MA PT license and home health experience are preferred.

Qualifications

• Degree from an accredited Physical Therapy program.
• Valid MA Physical Therapist license.
• Minimum of two years of experience; home health preferred.
• Current BLS CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Deliver physical therapy services per physician plan of care.
• Assess functional status including home environment.
• Perform assessments for strength, mobility, gait, and ROM.
• Administer PT treatments and instruct on use of devices.
• Report patient changes to physician.
• Manage PT programs and collaborate with care team.
• Instruct family and caregivers on treatment goals.
• Provide PT consultation to home families/caregivers.
• Conduct in-service education for staff as needed.
• Supervise PT assistants as required.
• In sole therapy, supervise home health aide care bi-weekly.
