Job summary

Qualifications

• Licensed Physical Therapist in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs in home health.
• Develops a therapy plan of care and implements interventions to improve mobility and endurance.
• Observes and reports patient responses and progress to the supervising nurse/physician.
• Instructs patient, family, and care team on plan of care and progress toward goals.
• Completes evaluations and progress notes in accordance with policy and state guidelines.
