Requires state PT licensure, valid driver's license or reliable transportation; CPR certification preferred within 90 days. Compensation includes annual total cash target pay ranges $104,840–$157,259, plus per-visit and hourly pay; full-time schedule.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing PT, per the plan of care, and in coordination with the care team.
• Observes and reports patient response to treatment and changes in condition to supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
• Ability to function in home settings irrespective of patient demographics.
