Job summary

Prismhr Hire is seeking an experienced PD Training Nurse to conduct in-home peritoneal dialysis education and personalized training with patients and caregivers. Training occurs in the patient’s home, promoting learning in a private setting while delivering clinical support.

The role emphasizes independence, organization, and strong communication skills to safely guide patients through PD procedures and self-management. Some travel to service areas is required.

Qualifications

• Current, unrestricted RN license in the applicable state(s).
• Minimum of 2 years of clinical nursing experience.
• Previous peritoneal dialysis experience required.
• Demonstrated knowledge of PD procedures, aseptic technique, infection prevention, and PD troubleshooting.
• Strong patient education and communication skills.
• Ability to work independently with minimal direct supervision.
• Valid driver’s license and reliable transportation.
• Ability to travel to patient homes throughout the assigned service area.

Responsibilities

• Provide comprehensive, one-on-one PD training in the patient’s home.
• Develop individualized training plans based on patient and caregiver learning needs.
• Train patients and caregivers on manual exchanges and/or automated PD (cycler) therapy as prescribed.
• Educate patients on PD supplies, equipment, dialysate solutions, medications, and treatment procedures.
• Assess patient/caregiver competency throughout the training process.
• Coordinate with nephrologist and home therapy team as needed.
• Complete accurate documentation of all training and home visits.
• Maintain compliance with CMS and state regulations.
