Job summary

Success Matcher Recruitment is seeking a Registered Nurse to join our client in Indianapolis as a Peritoneal Dialysis (PD) Home Training Nurse. You will educate patients and care partners, manage a home PD caseload, and coordinate with nephrologists to adjust care plans.

Responsibilities include home visits, safety assessments, troubleshooting PD equipment, and promoting transition to home-based modalities, enhancing patient independence and quality of life.

Qualifications

• RN license active and unencumbered in Indiana.
• Minimum 2 years of RN experience.
• At least 12 months of PD experience preferred.

Responsibilities

• Educate patients and care partners on PD mechanics, aseptic technique, catheter care, and cyclers.
• Manage a caseload of home patients, coordinating monthly visits and lab reviews.
• Conduct home visits to ensure a safe, clean environment for home treatments.
• Provide guidance on complications and equipment issues as a clinical resource.
• Educate in-center patients on benefits of home-based PD and transition to home modality.
