Job summary

Renviva Management Services, LLC is seeking an experienced PD Training Nurse to join our Concierge In-Home Peritoneal Dialysis Training Program. You will deliver individualized PD education and hands-on training directly in patients’ homes, enabling safe, independent dialysis in a private, comfortable setting.

The role requires clinical expertise in PD, aseptic technique, and infection prevention, plus strong teaching and communication skills.

Qualifications

  • Current, unrestricted RN license in the applicable state(s).
  • At least 2 years of clinical nursing experience.
  • Previous peritoneal dialysis experience required.
  • Knowledge of PD procedures, aseptic technique, infection prevention, and PD troubleshooting.
  • Strong patient education and communication skills.
  • Ability to work independently with minimal direct supervision.
  • Valid driver’s license and reliable transportation.
  • Ability to travel to patient homes throughout the assigned service area.

Responsibilities

  • Provide comprehensive, one-on-one PD training in the patient's home.
  • Develop individualized training plans based on patient and caregiver learning needs.
  • Train patients and caregivers on manual exchanges and/or automated peritoneal dialysis (cycler) therapy as prescribed.
  • Educate patients on PD supplies, equipment, dialysate solutions, medications, and treatment procedures.
  • Assess patient/caregiver competency throughout the training process.
  • Adapt teaching methods to accommodate literacy, language, learning style, physical limitations, and cognitive needs.
  • Reinforce patient independence while maintaining safety and adherence to the prescribed treatment plan.
  • Perform comprehensive nursing assessments during home visits.
  • Assess the patient's ability to safely perform PD independently.
  • Monitor catheter exit site and tunnel for signs and symptoms of infection.
  • Assess fluid status, blood pressure, weight, edema, ultrafiltration, and other relevant clinical indicators.
  • Identify changes in condition and communicate concerns promptly to the nephrologist and home therapy team.
  • Recognize and appropriately respond to PD-related complications.
  • Teach and reinforce proper aseptic technique and infection prevention practices.
  • Train patients on connection/disconnection procedures.
  • Provide education regarding drain issues, low drain alarms, slow flow, fibrin, constipation, and other common PD troubleshooting concerns.
  • Provide education related to cycler operation and troubleshooting.
  • Teach appropriate response to contamination, accidental disconnection, and other treatment-related emergencies.
  • Reinforce procedures for managing suspected peritonitis and other complications according to program protocols.
  • Ensure patients understand when and how to contact the PD clinical team.
  • Conduct scheduled training and follow-up visits in the patient's home.
  • Provide a professional, respectful, and patient-centered experience during every visit.
  • Evaluate the patient's home environment for safety and suitability for PD.
  • Identify barriers to successful home dialysis and collaborate with the interdisciplinary team to address them.
  • Assist patients with organization and storage of PD supplies and equipment.
  • Coordinate additional training visits when competency has not been demonstrated.
  • Provide post-training follow-up and reinforcement as directed by the home therapy program.
  • Complete accurate and timely documentation of all training and home visits.
  • Maintain competency checklists and training records.
  • Document patient/caregiver progress toward independence.
  • Maintain compliance with CMS Conditions for Coverage, infection prevention standards, and applicable state and federal regulations.
  • Participate in QAPI activities related to home dialysis, PD outcomes, training, infection prevention, hospitalization reduction, and patient retention.
  • Maintain patient confidentiality and comply with HIPAA requirements.
