Job summary

Angels of Care Pediatric Home Health is seeking a licensed Pediatric Physical Therapist (PT) to provide in-home therapy across Fort Worth, TX and surrounding areas. The role focuses on children with developmental and neurological needs, with flexible full-time or part-time options.

You will evaluate, design and implement treatment plans, collaborate with families and care teams, and maintain documentation in an electronic medical record.

Qualifications

• Active, unrestricted Physical Therapist license (Texas).
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• At least 1 year of experience working as a Physical Therapist.

Responsibilities

• Provide evidence-based pediatric physical therapy in the home and community.
• Perform evaluations, assessments, and ongoing progress monitoring.
• Develop and implement individualized treatment plans and goals.
• Deliver interventions to improve mobility, strength, balance, coordination, and motor development.
• Educate and empower families and caregivers for carryover in daily routines.
• Collaborate with physicians, caregivers, and interdisciplinary teams.
• Document care in an electronic medical record system.
• Maintain compliance with all clinical and regulatory standards.
