Job summary

PowerBack Rehabilitation in Laconia, NH is seeking a Physical Therapist to provide in-home outpatient rehabilitation for older adults, including visits to private residences and independent living facilities.

You will evaluate, design and implement personalized therapy programs, educate patients and caregivers, supervise staff as needed, and coordinate with a rehabilitation team to promote independence and safety.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice
• Master’s degree in Physical Therapy or related field with PT degree
• Bachelor’s degree in Physical Therapy with 5 years of PT experience
• US driver’s license or reliable transportation

Responsibilities

• Provide outpatient rehabilitation in in-home settings for older adults
• Evaluate, develop, and deliver individualized PT programs
• Educate and train patients and caregivers in necessary skills
• Supervise PT assistants and students as needed
• Maintain timely documentation and adhere to scheduling
• Promote safety and adapt plans of care for each patient
• Order supplies and equipment as necessary
• Collaborate with rehabilitation team to optimize patient outcomes
