Powerback Rehabilitation to You is seeking a licensed Occupational Therapist for in-home outpatient services in Bedford and Manchester, NH. You will evaluate and treat older adults in their homes or in independent living facilities, developing treatment plans focused on functional goals.

Qualifications

• Bachelor’s degree in Occupational Therapy (ACOTE accredited)

Responsibilities

• Provide outpatient rehabilitation services in home settings for older adults.
• Supervise OT Assistants and students where applicable and per licensure.
• Develop and document individualized treatment goals and plans of care.
• Assess patient needs, progress and modify plans accordingly.
• Order supplies and equipment; maintain documentation and billing records.
