Qualifications

• Requires a Master's degree in Physical Therapy and active PT license in the U.S.
• Experience delivering outpatient or home-based physical therapy.
• Must have strong patient education and documentation skills.

Responsibilities

• Provide outpatient rehabilitation services in home-based or clinic settings.
• Develop and modify patient treatment plans; document progress.
• Educate patients and caregivers on exercises, safety, and independence.
• Coordinate with rehab team and refer to other disciplines as needed.
