Job summary

Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist to assess and treat patients in a home health setting. You will focus on improving daily living skills and functional independence for individuals with varying conditions.

As part of a collaborative team, you will develop individualized plans, monitor progress, and adjust interventions to maximize quality of life for patients receiving home-based care.

Qualifications

• Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist/OT in the state in which he/she currently practices.
• A minimum of one-year experience is preferred, but can be less with Regional Vice President and/or Regional Director of Therapy Services approval based on prior experience setting.
• Medicare home health experience is preferred.

Responsibilities

• Help patients who have debilitating conditions improve the functions of performing everyday tasks in the home.
• Develop treatment plans to restore functional abilities and daily living skills.
• Assist patients with compensating for loss of function and adapting activities at home.
