Job summary

Select Home Health in Indiana is seeking a dedicated Occupational Therapist to join our in-home care team. You will assess patients’ functional abilities, develop individualized plans, and deliver therapies to improve self-care and independence.

You will supervise OTA, collaborate with an interdisciplinary team, maintain documentation, and participate in quality initiatives while following ACOTE‑accredited education and Indiana licensure requirements.

Qualifications

• Requires NBCOT certification.
• Indiana license as an Occupational Therapist is required.

Responsibilities

• Provide occupational therapy services per physician's plan of care.
• Evaluate and assess patient functional abilities and develop individualized care plans.
• Deliver therapeutic interventions to improve function, self-care, and independence.
• Monitor patient response to treatment and communicate changes to the physician and care team.
• Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive equipment.
• Reassess patient needs and revise plans of care as appropriate.
• Maintain accurate, timely clinical documentation in compliance with agency policies.
• Collaborate with interdisciplinary team members to ensure high‑quality care.
• Participate in quality improvement initiatives, staff meetings, and ongoing education.
• Provide required supervision of the Occupational Therapy Assistant (OTA).
