Aging Well Health Care, LLC is seeking a licensed Occupational Therapist to provide in-home occupational therapy on an intermittent basis under physician orders and the plan of care. You will perform initial evaluations, develop treatment plans, and document patient progress.

The therapist will travel to patient homes, collaborate with the Director of Therapy, and supervise support staff as needed. Two years of experience is preferred and licensure in the relevant state is required.

Qualifications

• Graduate OT curriculum accredited by AMA/AOTA and/or certified by National Certification.
• Eligible for National Registration Examination.
• Currently licensed in the state(s) where practicing.
• Two years experience preferred.

Responsabilités

• Implements prescribed occupational therapy.
• Directs and supervises personnel as required.
• Takes initial history and performs initial evaluation.
• Writes progress notes and reports to physicians.
• Instructs patients and families in home programs.

Connaissances

Two years experience preferred

Home health experience

Formation
