Job summary

Humana Inc. seeks a Home Health Occupational Therapist to assess patients in their homes, develop retraining programs and coordinate with physicians and the care team to improve function and independence. The role requires OT licensure, CPR, and a valid driver's license with reliable transportation. Prior home health experience is preferred.

Qualifications

• Current OT licensure in good standing.
• CPR certification required.
• Valid driver's license, auto insurance and reliable transportation.
• Preferred: minimum six months of OT experience.
• Home Health experience preferred.

Responsibilities

• Assess and screen daily living/work-related skills with retraining programs.
• Interpret functional tests to assist physician in evaluating function.
• Confer with physician and care team to revise Plan of Care.
• Provide therapy per physician orders to improve strength, coordination and function.
• Document care observations and progress thoroughly, and report to supervisor.
