Job summary

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings.
• Confer with the physician and health care team and participate in Plan of Care development and revision.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function per physician orders.
• Supervise and evaluate OT Assistant and Home Health Aide performance.
• Document patient observations, interventions and evaluations thoroughly.
• Participate in care coordination and discharge planning activities.
