Job summary

Humana Inc. (CenterWell Home Health) seeks a Home Health Occupational Therapist to deliver in-home rehabilitation, assess patients in living environments, and collaborate with physicians to plan care.

You will help patients regain independence with strength, mobility, and functional training, while ensuring safety and quality of care. You’ll work 40 hours weekly in a patient-facing role, with requirements for OT licensure, CPR, and a valid driver's license.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
• Review/expand retraining programs in line with behavioral goals of each patient and rehabilitation program guidelines.
• Design community reintegration activities to assist the client in physical reconditioning and coordinate with the interdisciplinary team.
• Report patient status and progress to the physician and care team.
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel.
• Supervise, instruct and evaluate staff performance in implementing occupational therapy services.
• Accurately document patients' care observations, interventions and evaluation.
