Job summary

Humana Inc. – CenterWell Home Health is seeking a licensed Occupational Therapist to deliver in-home rehabilitation, assess function, and develop plans of care that promote independence and functional recovery.

You will screen patients, interpret tests, coordinate with physicians, and educate patients and families while complying with physician orders and clinical guidelines. A valid OT license, CPR, and driver's license are required.

Qualifications

• Current and unrestricted OT license is required.
• CPR certification is required or equivalent.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess/screen patients’ daily living activities and develop rehab plans with measurable goals.
• Administer functional tests and interpret results to assist physician in evaluating function.
• Coordinate with physicians and rehab team on Plan of Care updates.
• Provide therapy and instruction to improve strength, coordination, ROM and function.
• Document patient progress and discharge summaries accurately.
• Educate patients and families; participate in care coordination and discharge planning.
• Contribute to community reintegration and adaptive equipment planning.
