Job summary

Humana’s CenterWell Home Health seeks an in-home Occupational Therapist to assess, plan and implement rehabilitation programs for patients in their homes. The role focuses on improving strength, mobility and independence through personalized therapy under physician orders.

The OT will collaborate with physicians and the interdisciplinary team, document progress, and contribute to discharge planning while maintaining patient safety and quality of care.

Qualifications

• Current and unrestricted OT licensure.
• CPR certification is required.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess and screen a patient’s daily living and work-related skills and develop retraining programs.
• Administer and interpret diagnostic and prognostic tests of function at home.
• Confer with physicians and health-care team to develop and revise the care plan.
• Provide therapeutic treatment to improve or restore strength, coordination and function.
• Review and adapt retraining programs to match patient goals.
