Job summary

Humana Inc. through CenterWell Home Health seeks an Occupational Therapist to support patients at home, helping them regain strength and independence. You will assess function, create retraining plans, and coordinate with physicians and the rehab team to optimize care and outcomes.

Responsibilities include documenting progress, supervising staff, and contributing to discharge planning while ensuring safety and quality in home-based rehabilitation services.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess daily living/work-related skills and develop therapeutic retraining programs.
• Administer and interpret functional tests to assess patient status.
• Collaborate with physicians and rehabilitation team on the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, and range of motion.
• Review and expand retraining programs to meet patient goals.
• Design community reintegration activities and educate patients and families.
• Document care observations and treatment plans accurately.
• Supervise OT staff and home health aides as needed.
