Job summary

Humana, Inc. is seeking a Home Health Occupational Therapist to deliver in-home rehabilitation, assess functional abilities and design retraining plans.

Qualifications

• Six months of occupational therapy experience preferred.
• Home Health experience is a plus.
• Current and unrestricted OT licensure is required.
• CPR certification is required.
• Good organizational and communication skills are essential.
• A valid driver’s license and reliable transportation are required.

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop retraining programs with measurable objectives.
• Administer and interpret functional tests in home settings to assist evaluation of function.
• Collaborate with physicians and the care team on the Plan of Care.
• Provide treatment to improve/restore strength, coordination, ROM and function.
• Document patient progress and outcomes accurately and timely.
• Coordinate discharge planning and support care management activities.
• Design community reintegration activities and adapt equipment as needed.
• Supervise OT assistants and home health aides performing therapy services.
