Job summary

Always Best Care of Greater Cleveland is seeking a licensed Physical Therapist to provide in-home therapy across Ohio. You will evaluate patients, create personalized treatment plans, and guide strength, balance, and functional movement in a one-on-one setting.

Responsibilities include hands-on therapy, progress monitoring, and educating patients and families for continued exercises at home. Travel to patient homes is required; CPR and state licensure are mandatory.

Qualifications

• Must be licensed as a Physical Therapist in Ohio.
• MS/DPT in Physical Therapy or BS with equivalent qualifying experience.
• Successfully completed the required state licensure examination.
• Current CPR certification.
• Committed to continuing education and professional development.
• Strong communication and interpersonal skills.
• Able to work independently with minimal supervision.

Responsibilities

• Evaluate patients and identify their physical therapy needs.
• Develop treatment plans based on each patient's condition and goals.
• Work on strength, mobility, balance, coordination, and functional movement.
• Provide hands-on therapy and therapeutic exercises.
• Monitor patient progress and adjust treatment as needed.
• Teach patients and families exercises and techniques for home use.
• Assist patients in safely moving around their homes and communities.
• Recommend equipment and mobility aids when needed.
• Communicate with physicians, families, caregivers, PTAs, and other care team members.
• Complete accurate documentation of evaluations, treatments, progress, and follow-up care.
• Participate in discharge planning and quality assurance activities.
• Supervise PTAs when required.
• Provide education to patients, families, caregivers, staff, and the community.
