Qualifications

• Bachelor's Degree required.
• BLS certification required.
• Licensed Occupational Therapist in Ohio.
• Valid state driver’s license (Home Care department).

Responsibilities

• Direct Patient Care 90-100%.
• Evaluates and treats patients after referral; develops and implements plan of care.
• Implements plan of care per OT framework: biomechanical, sensorimotor, rehabilitative.
• Reassesses progress and updates plan of care.
• Discharge planning with multidisciplinary team; instructs patient/caregiver; documents progress.
• Supervises OT/OTA staff and participates in department meetings.
• Educates and mentors OT/OTA students and other disciplines.
