Job summary

Responsibilities

• Assesses patient’s ability to safely perform independent living skills.
• Designs and implements the plan of care including type and frequency of treatment.
• Documents subjective and objective patient progress.
• Monitors and modifies the plan of care and seeks consultation as needed.
• Educates patient and family/caregiver about deficits.
• Plans/discharges patients from therapy services and setting.
